iPhone 5 Stopped Going Into Sleep Mode

Lately my iPhone 5 has stopped going into sleep mode, even though it's set to do this after 5 minutes. What seems to temporarily fix this is rebooting the phone, but then after a day or so it does the same thing all over again.

Any suggestions?
