Thought I'd help everyone out as I had this same worry. I decided to test it out on a working display iPhone at a local store. I used the tip of a paper clip. It scratches. Use a case of keep it away from sharp objects!

Sapphire is hard. On the Mohs scale of mineral hardness, the material clocks in at a 9 out of 10, while Gorilla Glass registers only a 6 or 7. The dense, crystalline material is so hard that few things in the world can actually scratch it, so it must be cut and polished using diamond, a 10 on the Mohs scale and the hardest mineral on earth.

There are other common materials that are harder than sapphire that can scratch sapphire. Sand actually often contains material that can scratch sapphire, and some modern building materials as well. Not just diamond.
Highly unlikely to scratch but still possible. As someone mentioned above, most luxury watches have sapphire crystals and while very scratch resistant, they can scratch. But it's very unlikely the camera lenses should get scratch under normal circumstances.
To sum up, sapphire can be cracked and scratched. But on the Mohs hardness scale (based on what scratches what), it is a 9 and max is 10. Only material harder to scratch than it is diamond.
Sapphire has average shatter resistance, so cracks are more likely than scratches. That said, because it's so small, it is extremely hard (impossible in daily use) to damage. Unless you are rubbing diamonds on it (engagement or wedding rings), you won't have any issues.

But I've never had this problem with previous phones and not my previous iPhone's either very unlightening it's not to do with protruding metal ring it has to do with the lens cover that covers actual lense sapphire might not be what it is made of so if it breaks 329 is cost and if not fixed dust will get in and actual clarity will be greatly impacted get a case or maybe just maybe it should of came with one since the design warrants oneProbably Apple wasn't feeling so great about sapphire in general after the debacle. I can't seem to find exactly how far out the camera lens protrudes. Does anyone have an idea? .6mm? 1mm?
My iPhone 6 plus lens cover which covers the lens has gotten broke somehow? Unknown to me how Apple can't replace lens cover its attached to back of case. 329 was the quote to replace entire phone even thou the camera works fine for now(dust will get in thou) soon# still works what's up with that??It is not made for use without case at all. I was sadly waiting for otterbox preserve but hasn't come out yet. No easy fix tape blurs photos I think this is very bad and sad ,maybe iPhone should state needs case immediately will break without warning since camera protrudes. I know it needs a case but this is terrible part of its design buyer beware!!!
