iPhone Call Forwarding App

I have a Vonage line and i want to forward my calls to my iphone. but on the display i want something to let me know that person whose calling is calling my vonage phone line. Instead of it just coming up with just a random number.

Is there some type of app for that? Or is it all just on Vonage's end. I've tried the Vonagent App but it didn't do what I needed it to do.
