I found a few videos and sites saying how to downgrade from 4.0.2 to 4.0.1, but I'm guessing this would only work if the phone has been previously jailbroken?

Yes, if you were previously jailbroken and saved your SHSH blobs with Cydia, then you can downgrade. Otherwise, no.
 
I tried the TV out cydia app with netflix.

Works great but after a couple of minutes the picture just freezes. (both on 3G and wifi....)
 
With an AV Cable and iPhone, I'm not sure why most would bother with the Apple TV.

Air Video Server on the Mac and the client app on the iPhone means the iPhone can actually play EVERYTHING you have on your TV while the Apple TV is a big pain to deal with when using lots of video formats.
 
1) Netflix app is updated for Video out on the iP4 and Touch 4G. So to those yesterday saying it wasn't today it is available.

2) Any iDevice < iP4, iPad or Touch 4G can't do video out(no for 3GS users) , so OP the cable you have wouldn't matter, Netflix only enabled video out for those 3 devices I list right above!
