I set my child's downtime from 7pm to 7:20am, and this morning I see that somehow my child was able to use the iPhone for half an hour from 6am to 7am. How is that possible? "Always Allowed" apps are set to just Phone and Maps.

First thing to do is change the passcode.

Speculation but is it possible to change date & time during downtime?

The only true way to have downtime is take it away. They find ways. Mine are almost fully grown so we’re past it but they found ways over the years.

Venturing a guess, might be same bug that allows downloading more than 150MB apps over cellular (thread on this sub board on it). Turn off automatic time setting, roll back the date/time. Reset back when done.

Turned on screen time and changed a couple of settings to "Don't Allow" and was still able to manually set date/time.

Like I said, a guess, as did not want to dive deep into Screen Time and Restriction settings.
