iPhone won't ring or vibrate on calls

For the last many days my iPhone 5 will not ring or vibrate on phone calls. Ive adjusted the volume and checked the setting. Any ideas?

Im Missing many calls due to this.

mute is off and do not disturb is turned off.

I plays music fine so its not the speaker
I get video with sound from NFL games
I get radio broadcast on the NHL app
I have even reset to factory settings.

my coworkers brand new iphone 5C stopped vibrating all of a sudden. it appeared to be the vibrate motor sticking. if you smack the back of the phone, it started working again. may want to look into that.
