In that case no it’s not normal, in the one pic it looks like the flash has bounced off something (hence I asked about a case) and sent a shimmer over the lens as it looks quite washed out in the blaring light spots, I’d get it replaced.Nope. Completely bare.

That’s not lens flare. It’s a reflection.
It is sort of normal in that it is a known behavior happening over several models. Supposedly it is due to the gap between the lens and the glass covering the camera(s).
I’m pretty sure it does affect every iPhone XS (and X and 8 and 7...), but it doesn’t show up in every picture. It’s under specific circumstances of light intensity and angle. Apple replaced the rear camera module.
