Issue in "Open With" contextual menu

Is anyone else seeing this? see attached picture.

In my open with menu, i get about 3 instances of the same application. So say i want to open a jpeg, illustrator will come up 3 times, preview etc etc.

Is this just my machine? or are other people experiencing this?
