Within iTunes9 all my artwork, without exception is correct. I embed the artwork to the files before importing and ensure all tags are correct.

Within the iPod on my iPhone 3GS 32GB, most of it is OK but a percentage is missing.

Does anyone know why? Is it related to the image format/size embedded in the files?

Would love to know so I can eliminate this loophole in my Iron Fist of Perfectionism iTunes library management, Jawohl!

i had this same problem after upgrading to itunes 9. I synced a load of other playlists to my iPhone 3GS, and a fairt few of the albums had missing cover art - they showed as white.
I just removed all the music from the iPhone (via a sync), and then did another sync with all the music i wanted checked again.. this seemed to solve the problem for me. :)
