macOS Little help please!

Im trying to create a program that will allow me (user) to input information into a NSForm(probably have 7 cells) which will then gather that information on a table with each of the categories.

Name
# of reps for Exercise #1
# of reps for Exercise #2
# of reps for Exercise #3
# of reps for Exercise #4
# of reps for Exercise #5
# of reps for Exercise #6

Anybody know how I could start?

Just as you requested, you're getting little help. You might have done better to state that you need NSForm help in the subject. A lot of people will avoid vague subjects.
