Is there a limit of only three lock screen widgets? I can only put one above the clock and two below.

Only one widget can be above the clock. You can put 4 small Lock Screen widgets below the clock or 2 small + 1 large or 2 large, like your current screen.
You will probably have to wait until iOS 18 to see improvements to the Lockscreen widgets. Next year they will add it to iPadOS and the year after, it’s time for optimizations. Apple is a small company with limited budget, they can only work on so much at a time 😉
What is more you can’t have:

But you are forced to have:

or

It's not a bug, it's a design choice. They won't make any changes until iOS 17 at the earliest or ever at all.
