Mac freezes in screen saver mode

Why does my 2012 mac mini freeze when it is in screen saver mode. I'm having to turn it off and on each morning or afternoon? I leave it on continuously, but have begun shutting it off and starting with a fresh boot each time. Any solutions or thoughts?
 
I have this problem in sleep mode also

I will attempt to wake up Mac Mini and nothing will happen until I pull the plug and plug it back in.
