I have a 2018 Macbook Pro 15 that appears to be getting throttled at random. This results in cpu speeds of ~1 ghz and very poor performance. This does not appear to be related to a thermal issue. Activity monitor shows my CPU getting hammered but the actual clock speed never goes up to deal with the demand. The only thing that appears to remedy it is restarting the computer.

Below is a video of the affected computer with activity monitor & intel power gadget open.

If an SMC reset does not help, if bring it in for service. There is something very wrong with that machine. Notice the discrepancy between CPU utilization between activity monitor and PowerGadget.
