Ok so this happened on my 3G and on my iPhone 4... basically i have it jailbroken with and i used make it mine to change the AT&T to SKENE. so i went to update my ip4 and i backed it up, updated, then restored from backup and its not jailbroken but still has "SKENE" on the top instead of AT&T... anyone else notice this? Do i have to restore as new phone to get rid of it?
 
I noticed that too. It seems that the only way is to remember to wipe it out before restoring.
 
You have to set the phone up as new to get rid of the name. The name gets backed up and restored when restoring from the backup.

This is a known problem I've had this issue before. All you have to do is download MakeItMine again and you can clear it within the app.

you are correct, MIM is incorporated in the back up, you can do 1 of 2 things
Before backing up revert back to stock carrier or restore as new phone
