? mark on top left side of a window in leopard

I have this situation that just occurred recently.

In every window that I open, it shows a ?.
If I click on it , I got the message item not found.

Why is this? 😕

How can I repair this issue.

You probably made or accidentally made an alias to a file or app at some point and that file or app is no longer where it used to be. Hold down the apple key, then click and drag the question mark off your window to delete the busted alias.
