MBP 2011 - Missing Icon

Any ideas on this? (re: images for missing icon)

Whenever I click on the question mark, a pop up window with "The item cannot be found" appears.

It's probably just a file you once accidentally dragged up there and then deleted. You can just drag it off while holding the command key.
