Hey all, I have a 2012 13" MBP (non-retina) and I've gotten this glitch a couple times over the 6 months I've had it.
2.5 ghz ivy bridge
16gb 1333mhz ram
Running 10.8.2

What did this end up being? It'd be nice if people posted the results of things like this and not just let the thread die.
 
That is a dead (or dying) graphics chipset.

Spoken with such confidence. And yet, entirely wrong.

I've taken the following steps to narrow down the issue:
Tightened the monitor cable
Update to 10.10.1
Apple Hardware Test (Failed)
Wait, why would the hardware test fail? Black bars?
Okay I tried taking a stick of ram out from the top slot.
Still AHT failed.
So I took the bottom ram piece out and put in one stick to top slot.
AHT passed.
Bottom ram slot is dead.
