MBP Volume Fluctuates Randomly

Hi,

I've searched the forums regarding volume control being altered without hitting any keystrokes on the keyboard. Already have tried resetting the PRAM and I am still experiencing this issue.

Are you using the internal speakers, headphones, USB, or a combination?

I notice that when I change from external speaker to my headphones, the volume suddenly set to be very loud (although the volume indicator tells me that it's at minimum (1 bar)) So I need to press the volume button up and down several time before it goes back to normal when I use headphone.. Is there anyway to fix this?
