Merged Apps? Have you seen this?

Has this ever happened to anyone before? Note how the Remote icon looks like Trapster. However, when I tap the remote it works as remote not trapster. This happened when I restarted my iPhone, and can't seem to get it fixed.

Just hit reset home screen layout and it should be all fixed. It's happend to me 3 times now.
