Just noticed this yesterday. I installed a new application and the icon was missing after it installed. The same thing happened today when I installed Google Chrome. The name is there but the icon is missing. This happened earlier last year on another iMac I had. The solution was provided by the company with the app, but obviously it is OS X that is the problem. Anyone know how to restore the default icons?😱
