I have a jailbroken iPhone 4S on iOS 6.1 and tried to restore the phone through iTunes but it failed and I'm now missing the Cydia app. I've tried re-jailbreaking but that didn't work. Anyone have any ideas on how I might get the Cydia app back?

Are you sure the restore wasnt successful? what was the error btw? And Did you tried searching cydia through Spotlight'?
 
I keep getting error 3194. I just want to restore my phone and lose the jailbreak.

Error 3194 is when you are trying to restore to a FW currently unsigned by apple ... if you want to unjailbreak you can always restore and update to 6.1.3

Settings -> general -> reset -> reset home screen layout

Assuming you're still jailbroken, you will see the cydia icon
