I just noticed that I'm missing my facetime app in my iPhone 5. My iPad mini and 4s has it though. Not sure if it started with beta 4 but I'm sure it was there with beta 3. Under settings its also gone. Anyone else having this issue?
 
No, it's nit hidden. Did a search too but it's really no where to be found. When making a call its totally blank from the call menu. Nothing in settings at all. ImageUploadedByTapatalk1376046638.442018.jpg

Check if FaceTime isn't disable in Restrictions. (Settings / General / Restrictions)
If it's disabled everything related to the app (icon, settings menu, etc) will be hidden.
