My new iMac is a pain in the neck

I have two Dell Height-Adjustable monitors hooked up to an M1 Mac mini. I can even put the monitors in Portrait mode though that would make life even harder for Progressive lenses. I bought the monitors a long time ago when they were reasonably priced. These monitors are at least double the price of two years ago.
I wonder if you could rig something up to mount the iMac upside down on a table.

There’s nothing Apple can do that wouldn’t have some portion of users complaining about it!
It's not a "flaw" and most people have no major issue with it. It would be one thing if the OP said, "I would like to buy an iMac but I can't because the stand isn't adjustable." But instead he just buys it and then complains about it. That would be like me buying a car with basic forward-backward adjustable seats and then complaining that I can't adjust the height of the seats 🤦♂️ It's not exactly a secret that you only discover once you get the thing.
And on top of that, you can buy the iMac without the stand and with a VESA mount adapter instead:

Well, in this case, I agree with some people here, get yourself a Mac Mini.. you can customize it to the point it's identical to the new iMac. I mean they both use M1, right? Unless you really want the color-matching accessories and the shiny magnetic power cord, Mac Mini gives you endless possibilities about hand-picking all your peripherals to any price point, and adjustable height LOL.

return it and maybe get a Mac Mini with a monitor that can be adjusted. or maybe a macbook.

I have presbyopia and I know what you mean. My progressive glasses are designed for far and very near and are specifically for driving with. I cannot use them for computer. Perhaps your progressive lenses are like mine. Go to your optometrist and describe the problem. There is a specific progressive lenses designed for office/computer use, where the intermediate zone is wider, and the far and reading zones are wider.
I have a pair of tri-focals like that but I rarely use them because the other two distances are so small. Single-lens and bi-focals cost a lot less than progressives and you can just leave them at your computer desk.
I can’t image the iMac being too high. My problem is it’s too low for ergonomics which requires a riser of a few inches. Does your desk have a super high riser or something?
Nah. It's a fault of the iMac. You shouldn't need to order a special model to put it on a decent stand nor should it come with a worse stand than a $200 Dell monitor for $1200. Monitor ergonomics is important and Apple has been shipping horrible stands for over 15 years now.
The vast majority of monitors these days come with at the very least holes for a VESA mount allowing the use of a wall mount, monitor arm or 3rd party stand. The iMac should be no exception. You shouldn't need to decide at time of purchase whether you need a VESA mount.

One shouldn't need a totally different model of the same computer just to do VESA mounting.
It's totally ridiculous - particularly in an all new clean sheet design.
Yes switch to a pair of dedicated computer glasses for close up work like tons of other people do, I could never imagine having to use bifocals for computer work. Bifocals are good for ocasional close up work, but if you are going to stare at a computer for a long period of time might as well have a good pair of glasses that covers your whole vision. Plus then you can sit of straight and view the computer screen head on, way better for your posture than a low screen.
If you don't wear progressives, you completely wrong. If you do, then you know you can't use any monitor or iMac with out eye strain.

You need an adjustable desk...there are some affordable options if you're not looking to spend $800 on a nice one.
But I got an adjustable desk during the pandemic and I can never go back to something that isn't adjustable.
Both. A lot of people get comfort from complaining about things. The only difference is we don’t see posts here from the “real life” group. Well, I mean, unless we are ALL bots, we’re all members of the “real life” group, too, but you know what I mean
Remember people are different sizes. I know I couldn't use the current iMac, it's way too high..
Ergonomics are up to the user to adjust to or remedy the situation. No manufacturer can make a product that suits everyone perfectly. When I got my 2014 VFR800 (see my avatar), I had to lower the foot pegs 1" and raise the handlebars an 1 1/4" to suit my aging physique. I didn't call Honda names and whine a bleet about it, it was MY problem and I fixed it, 40,000 miles later it is still as comfortable as ever, and I push hard it through the canyons and twisties. I just ordered my new iMac, and I don't see how it is that much different ergonomically that my current iMac.
