newest iTunes duplicating my playlists

Why are all my playlists in iTunes being duplicated. I don't know if it has to do with my iPhone/iPad when syncing, not sure what's going on. It's really annoying though right now I have 3 of the same playlists in my iTunes. I noticed it happening in the most recent iTunes and never before.

How do I stop this from happening?
