All the way up, and no, not in a noisy environment. It was worse under 2.2.1.

Edit: What is strange though is that my alarm is loud as hell!

I wonder if you have a clear screen cover on it? It would block the regular phone speaker (on top), while not obstructing the bottom left grill speaker, which is the one your alarm and music come from, which is why the alarm is loud
