Option and Command icons floating on iPad screen

My dad sent me some screen shots of his iPad. For some reason, icons for the Option key and Command key are floating on the top right of his iPad’s screen. If he reboots, they go away, and then come back on their own. I’m guessing some weird accessibility feature must be turned on, but I can’t figure out which one. This makes no sense.

I’ve googled the hell out of this and don’t see anyone else talking about it.

That should be Settings/General/Accessibility/Keyboard/Sticky Keys
Select Sticky Keys Off.

That should fix, I think.
