You just need to be on iOS 6 to get the OTA update. Go to Settings, General, Software Update.

I did my iPhone and iPad that way today and it worked fine. 🙂

Hard to say. I'd try restarting the device and attempting it again. If that doesn't work, connect it to your computer with a cable and see if iTunes will update it.

If you have an iPhone 5, then yes. Otherwise, then no.

I had no problem here in the UK updating an iPhone 5 and an iPad.
