Verizon allows it on all Share Everything and prepaid plans. I believe T-mobile, too, and reports are it even works with the 200MB free data.
AT&T allows it on all Mobile Share plans and the 5GB/$50 prepaid plan.
No idea about Sprint.

Yet another confirmation for T-Mobile here. Just received my Micro SIM from T-Mobile last night. Popped it in my Verizon iPad 3 and activated with the 200MB free data and Personal Hotspot is working just fine. Granted, it's not like I'll be doing a lot of tethering with it on just 200MB of data.
Yep. If you have Verizon's Share Everything plan, you're good to go for no additional cost. Unless of course you go over your data plan.

Do not need share everything. I do not have it and my iPad Air has the Hotspot and I have used it right out of the box.

AT&T allows iPad hotspot with the $50 plan.
I called AT&T yesterday and confirmed they will not enable the hotspot if you have the grandfathered iPad unlimited data plan.
