This is the second email like this that I received in the last month. This phone (my wife's) was not set as the trusted device...my other phone was the only one. Her phone is on IOS 6. After the first email I reset the iCloud password. Any ideas as to why this notification is being triggered?

    Dear xxxx,

“xxxx's iPhone” was automatically removed as a trusted device from two-step verification for your Apple ID (xxxx@xxxx.com).

Your trusted devices are used to verify your identity when you make changes to your account, sign in to iCloud, or make iTunes or App Store purchases from a new device.
To protect your security, when you erase a device it will be removed from your list of trusted devices. To re-verify your device, sign in to My Apple ID choose Password and Security, and then Add or Remove Trusted Devices.

If you did not erase this device or if you believe an unauthorized person has accessed your account, you should reset your account password immediately at My Apple ID.

I have received this email too in some cases when I didn't expect it (e.g. when activating Find my iPhone). I think there are some glitches. Just check on appleid.apple.com which devices are really registered as trusted devices.

Thanks. Now that I think about it, I was using Find-my-Phone a day prior to receiving the emails.
