Photos blur with flash on?

As you will see from the attached, any photo taken with the flash on leaves 3 blurrys type light circles, any idea how to fix this?

Look closely, is your lens dirty?

Also, I'm not sure if this makes any difference, but do you have a Black iPhone or White?

Are you using a case or skin? Might be reflecting off that.

Wondering would a bumper be better than a case then? Blurry photos are no good.
