The theme is Muze 2, RASBerry and HideMeX to clean up the status bar, and here is the wallpaper...

From DaRock posting on modmyi - the first one is from Votive and the second one is from Playdough. Both are paid themes, but Playdough's in beta still.

The theme is Miui 6 for iOS 8, although I added a few icons and had to tweak the calendar plist. Also, the lockscreen and iwidget are both from Schnedi (with a bit of extra tweaking).
