Hmm, good question. I'm going to be a gimmick that probably works on the 4S but they won't allow it...just so people can buy the 5.
But I'm going with...I honestly can't think of anything yet. Maybe more visual effects in iOS 6 for the new iPhone since the hardware in the 4S "supposedly" can't handle it.

A 16 megapixel camera with 4K video recording at 30 frames per second and 720p HD video recording at 120 fps with a special "iSlowmotion" app - and you can use it under water.
And psssss, you left out two generations: the first one (simply Apple iPhone (also known as "iPhone 2G")) and the third one (iPhone 3GS).

I don't know how you can say "except the bigger screen and LTE"? Those are the selling features. Here's what I'm guessing the next 2 years of iPhone hardware will look like:
iPhone 5 = LTE, 4" retina screen, revamped Siri
iPhone 5s = 802.11ac
I expect Apple to release new Airport Extreme with 802.11ac this fall in preparation for the next iPad and Macbook refreshes in 2013. Then they can use that feature in the iPhone 5s next year. It's a lot easier to stuff a new chip into the iPad and Macbook and then figure out how to stuff it into the iPhone.
I said that because those features are expected...im looking for the "one more thing"..."Siri" feature. the one that isn't pointed to and will be a surprise.
I think there would be outrage if the next iPhone turns out as you've said above.

Although it's highly unlikely on the next iPhone, I can't wait for NFC.
You know that when Apple releases it, every other hardware manufacturer will do so as well, and then it will finally force the technology to become mainstream. The applications that will be driven by NFC are the coolest to look forward to! Mobile payments, keyless locks, data exchanges between nearby mobiles, etc...
With the future evolution of Siri, backed by strong NFC applications, we will have great leaps in how we do things in our regular lives.
