Is there a way to prevent apps from reopening after the system crashes and/or when you install updates that require reboot? I hate the default behavior and always have to reboot twice just to get a clean desktop.

The dialog that asks you if you want to "Reopen windows when logging back in" has no effect on crashes/update reboots.
 
Sometimes - if you have the application open - go to the "Preferences" option in the Menu there can be settings regarding the opening or closing of apps.

Otherwise, go to the application's icon in the Dock and right-click it and sometimes an options menu pops up giving you the option whether you want the application open at startup.
 
No, you can only disable Resume completely. It is meant for unexpected terminations.
