When my phone is on silent (to stop me being disturbed by social media, work etc ..) I have 1 app I still want to make a noise (the app that does my doorbell, my security cameras etc).

How would I set this up to allow this app to always disrupt me, but the others to not?
 
I don't think you can, there is an API for developers to generate critical notification alerts (which is what Apple calls a notification that can bypass silent mode) but there is a special entitlement they need to request and they would need to implement this functionality in their app. The best thing you could do would be do a Focus that allows just that app notifications (and then disable silent mode).

If you're using Ring, you can set up the notifications for your doorbell and other cameras to 'Priority Alerts' which the alert sound will break through the Silent mode and Focus modes.
