Water damage would be unlikely to make it hot.

Mine gets hot rarely, typically when it is charging, but that’s it.
 
My vanilla 11 was getting warm during charging after latest iOS update until I turned off optimized charging.
 
I’m using a regular iPhone 11 as my daily driver currently and it never gets hot or even warm with regular use. Unless you’re playing heavy games with full brightness on 4G, getting overly hot is definitely not normal
 
Last week. Some choc milk cleaned it off quickly. Gets hot after using. Can feel through case..
But again, more information is needed, such as what exactly are you doing when it gets hot. Streaming? Gaming? Give us an example of what type of usage gets it hot.

Water damage can create a short which can lead to all kind of side effects depending on where the short occurred.
Yup no kidding. I was asking why OP was concerned about water damage as he/she was very vague about the specifics regarding when his/her issue occurs.
