Problem replacing Application Icons

I've done this before without any issue, but now when I try to do it it just shows a preview application icon as the icon with the file extension on it. Any help?

It used to be easier to change icons. Now it doesn't always work as it should. I use Candybar which works great and keeps all the icons in one place.
