I am preparing a disk image of an application for distribution, and want to get it right.

I have added my app and an alias to /Applications with large icons. The background of the folder instructs the user to drag the icon to the alias. There is even a graphic in the background.

I have aligned the icons, and resized the folder before creating the disk image. However, when I open the .dmg, the icons are put in the top left corner, and in alphabetical order! How do I get them to stay put?

That worked pretty well.

I have the finder window that opened after mounting the resulting image attached as a JPG.

The basic steps were:
Create your folder
Create disk image that is read/write from the folder
Show View Options, choose the window only, and customize icon size, etc.
If you have a background image it can be hidden with SetFile
Unmount the image with the finder window where you want it to appear
Use disk utility to convert the image to a compressed image

How do you remove the Device and place lists on the left hand side and the menus up top and for just that one window?
