Apart from it heating abit more what really could be the issue

Itrucolor has lcd screen abit steep but apparently good quality?
 
It would not physically fit. That's one of the merits of AMOLED displays as they are very compact.

I'd sell your iPhone and get an iPhone with a LCD display like the iPhone SE.
 
Yes, pwm sufferer here. Switched my iPhone 12 to a “in-cell” lcd and no more headaches. Colors aren’t as good as before and maybe some extra lag but worth it for me. Have had the phone this way for a year now. Battery drain is pretty steep too, but the iPhone 12 battery is notorious. The Apple MagSafe clear case does a good job not hugging the screen, which now pops out a bit because lcd is thicker than oled.

I don’t think I suffer from PWM per se but looking at my iPad long time feels less stressful than looking at my iPhone XS Max, which has an OLED screen. Coincidence maybe? Or I just don’t like staring at tiny screen anymore.

With that being said, PWM is just engineer’s cheap and quick way to dim an LED source since incandescent light bulb dimming has nothing to do with PWM, thus friendlier to our eyes. (Can we also blame PWM to much higher prescription glass rate among teenagers nowadays? :D )

Pretty much every repair shop has replaced OLED with LCD whenever the customer wants a lower cost solution. Even LCD replacements have premium, standard, and low quality and resolution options to further drive down costs.

Doing such a thing may cause software issues on your phone due to Apple's propensity to program and partner individual components to individual devices. For example, you cannot switch the LCD panels of 2 identical M1/M2 MacBook Pros without running into serious glitching and artefacts on you screen as, despite being identical, the panels haven't been electronically paired.

I am not sure if Apple have pulled off the same anti right to repair practices on their iPhone line yet but if not, it'll only be a matter of time before they do.
