Question about Apple Battery Exchange Program

I've been looking at their webpage on this issue:

And I'm not sure I'm interpreting it correctly. Do I need to exchange my battery only if it falls under both the stated model number AND the stated serial number range?

Yes. Your battery has to be the right model number AND have a serial number within the given range in order to be exchanged.
