This is probably a dumb question but it's still unclear to me.

Will AC+ Theft & Loss only work for those who fully own the iPhone? Or does this also work for those of us who are on annual upgrade programs, such as AT&T Next?

Not sure about att but with the Apple IUP you can also add the Theft and loss if you desire.

I didn't notice it when I ordered my phones. Now Apple says I need to bring them in for a diagnostic test before I can add it. I'm kind of annoyed that I didn't see it earlier.

I think you can buy it regardless of where you bought it, just a matter of when. You will enter in your IMEI Number on the website and it will let you buy it.

It should work for any. With ATT you have to buy it outright and they may want you to come in to store or call Apple to diagnose it.
With IUP you are paying it each month - if you upgrade in the year, you've only paid half.

I just paid $9.99 per month for the AppleCare+. I never lose my phone and my phone already has theft protection. See pic:

You do Not have to go into the Apple store. They can do the diagnostic remotely. I have done this in the past.

I ordered mine from Verizon and signed up for AppleCare+ with theft/loss coverage online. They ran a quick, remote diagnostic and let me purchase. Pretty easy process. I didn’t read anything about coverage only applying to paid off phones.
