There's a big question mark in the upper right of all Finder windows:

What is that? Never seen it before.
 
It is a missing toolbar icon. The same happens when you have a Dock icon that no longer properly links to its target. It can be removed when you are in Customize Toolbar.
 
That means you dragged a file or folder up there at one time, creating a button for that folder, but you have since deleted the folder.

Just right-click in the toolbar, choose customize toolbar, and drag the question mark off the toolbar.
