In Save and Open dialogs, Command-D selects the Desktop folder. I'd prefer to have this shortcut select the Documents folder instead. Any way to reprogram that Keyboard Shortcut? I couldn't find it in System Preferences > Keyboard > Shortcuts.

It's CMD+Shift+D… There is in fact a 3d party tool to reassign shortcuts, however I can't recall its name. In the Meantime: The shortcut to the Documents folder is CMD+Shift+O.
