I have the iphone 4s and if I get a text message while I am on the phone it plays the incoming text alert. This is quite painful because it blasts it in my ear. I have gotten to the point where I make sure the phone is on vibrate before I answer/make any calls.

How do I fix this? What setting am I missing?
 
You can disable the text sound entirely in Settings->Sounds->Text Tone. However, I don't believe there's any way (with stock iOS) for it to intelligently disable the tone if you are on the phone.
 
If you flip the vibrate switch while on the phone it should vibrate instead of making the notification sound.
