Disclosure up front: I'm the developer of the app below. Risky Business is free, it will stay free, and there's a companion thread over in the Mac Apps forum – but the way this works on a phone/ is different enough that it seemed worth its own post here.
Why this belongs on a phone at all
Project risks are not discovered at a desk. They get mentioned halfway through a meeting, or by a supplier on a call, or by someone in a corridor who assumes you already knew. By the time I'm back at the Mac, half of them have evaporated. So the iPhone side of Risky Business is built around capture: open it, pick a category, set probability and impact, done. The analysis and the reporting can wait until you're sitting down.
On iPhone and iPad
On your device. No server of mine, no account to create, no tracking, no ads, nothing passed to third parties. Sync, if you turn it on, runs through your iCloud. Free, no in-app purchases, no subscription — I've had a lot of use out of other people's freeware over the years and I'm glad to give something back.

- Home Screen and Lock Screen widgets, Control Center, plus Siri and Shortcuts for capturing a risk without opening the app properly
- The full 5×5 matrix and the portfolio view with its priority heatmap, both usable on iPad
- Review cycles: every risk carries a "next review" date, with reminders on the dashboard
- Reports as PDF or PowerPoint straight from the iPad, with selectable sections and your own branding
- Excel, CSV and PDF export of the audit log; Excel import if you're coming from a spreadsheet
- Optional iCloud sync, so what you capture on the phone is on the Mac when you get there
- VoiceOver including an audio graph of the charts, Dynamic Type, Differentiate Without Color
- English, German and Spanish
