Long-time member here – I've had this account since 2009 and have mostly been on the reading side of it. This is the first time I'm posting about something I built myself, so, disclosure up front: I'm the developer of the app below. It's called Risky Business, it's on the App Store, and it's free.
Where it came from
I manage projects for a living. Every project I've been on kept its risk register in a spreadsheet, and every one of those spreadsheets was abandoned somewhere around week three – because updating it was tedious and because nothing useful ever came back out of it. Risky Business is my attempt at the opposite: a proper native app where entering a risk takes seconds and where a report a steering committee will actually read comes out at the other end.
What it does
All data stays on your device. There is no server of mine anywhere in this, no account to create, no tracking, no ads, and nothing handed to third parties. If you switch on sync, it runs through your iCloud, not through a backend of mine. Free, no in-app purchases, no subscription. And it will stay free. I've benefitted from a lot of freeware over the years, and it feels good to be able to give something back.
The caveat, up front
It requires macOS 26, iPadOS 26 and iOS 26. I know that rules some of you out, and I would rather say so in the first post than have you discover it on the App Store page.

- Capture risks with category, probability and impact, from templates; scores and priorities are calculated for you. Keyboard shortcuts and bulk editing throughout.
- A 5×5 risk matrix, with a Current → Target view so you can see residual risk rather than just the initial rating.
- A portfolio view across projects: key figures, a priority heatmap, and the risk budget per project.
- Risk budget: a calibratable mapping of probability and impact to monetary values, with an expected value per risk.
- Timeline and a complete audit log of every change; exportable as Excel, CSV or PDF.
- Review cycles – each risk carries a "next review" date, with reminders on the dashboard.
- Management reports as PDF or PowerPoint, with selectable sections and your own branding. Excel import and export.
- Multiple accounts, admin roles and per-project permissions, with optional AES-GCM passphrase encryption for shared databases.
- Optional iCloud sync and collaboration, with named storage areas per database. (with version 2.5 - pending for approval)
- Home Screen and Lock Screen widgets, Control Center, Siri and Shortcuts.
- VoiceOver support including an audio graph of the charts, Dynamic Type, Differentiate Without Color, and improved contrast.
- English, German and Spanish.
