Somehow the Safari icon name was changed to a document name. Now when I hover over the icon instead of "safari" I have the title of a document. I can't find a way to reset it, and I've no idea how I goofed this up.

Is this icon in your dock? If so, I would just take it and throw it in the trash. Then open Applications and drag a new shortcut of Safari to the dock

Yep just drag the icon to the trash. Go to your applications folder and click on Safari. You will then see it in the dock and then you can right click(control click) it and select "Keep in dock".
