Safari new tab favicons abnormality

Why does safari in the iPhone only display about 5% of the previously visited website favicons when you select the + button to create a new tab even though the website does have a favicon that displays perfectly well in other browsers.

There are standard "favicons" and then there are favicons that must be uploaded by the developer of the website that Apple looks for. The later is what's displayed on iOS Devices.
