Handoff is protocol that passes the current state among Apple devices. The benefit of having a camera on an Apple TV is that one can easily mount it where it makes the most sense, and know that it will always be there without needing to configure it each time.If you want camera as a separate accessory, it might as well be iPhone paired with Apple TV via Handoff.
It increases cost over an integrated box, but it increases the cost of the base product and makes it much less flexible. Having a camera would be nice for Video Conferencing, Games and Fitness. A bundle of a camera with a controller as a game focused package, another with sensors bundled for a fitness focused package and maybe one with both would be really cool, but by having it be separate, they can still keep the core Apple TV reasonably priced.Having said that, I don't think Apple TV is popular enough to be sold at $299 with built-in camera. But if it is marketed as a fitness device (e.g., Apple TV Fitness Edition), perhaps there will be more takers? As Peloton proved, when it comes to fitness, folks are willing to pay.
You should look into HomeKit-secure routers. That would simplify at least half of that scenarioThere are ways to secure a smart home. I have my Apple TV mounted to the back of my Sony TV, since the remote uses bluetooth. And I have any smart home devices (Including the Apple TV & HomePods) on a separate VLAN from my main network (so they can't communicate with my main network), as well as my firewall is setup to not allow traffic from the TV or cameras to reach the internet.

Well, it's not about the fee itself, but Samsung backing a competing (and IMHO sub-par) standard - also see VHS vs. Betamax. Prolly the biggest issue with their QLED lineup. I did. Not much out there except the Amazon-made eero, I don’t trust Amazon much. I work in IT, so I bought the Ubiquiti Dream Machine. Fantastic enterprise grade hardware, the ultimate all-in-one router for geeks like me...You should look into HomeKit-secure routers. That would simplify at least half of that scenario

Right, none of the Apple TV models offered webcam. I was merely responding to your "USB webcams" comment, which isn't possible on Apple TV 4K as it doesn't have a USB port. If tvOS adds support for webcam, I suspect it will be using iPhone as Continuity Camera or new Apple TV model with built-in camera.
