I just added a second monitor to my iMac and suddenly Back To My Mac/Screen Sharing is acting up when I log into it from my MacBook. I can move the mouse around for a moment, but when I move to the dock or try to select something from the menu, the shared screen freezes and I get a disconnect message. I've tried to only look at Display 1 but it never manages to switch the view before it freezes up, and when it reconnects it's always in dual-screen mode.

Any ideas?

Well, first you should at least just try mirroring the screen to see if it's a hardware problem. It's most likely a flaw in the software, though.

Same problem -- for me it was the external display

I had the same problem with my iMac -- and the external monitor was definitely the cause. Switching from extended desktop to "mirror displays" solved the problem instantly.
