My husband upgraded to iOS 12 and today he got a message that he reached half of his screen time. When he went into settings it is asking him for the passcode which he doesn’t know as he’s never set one up.

Any suggestions on how to recover what the phone thinks the code is? Or to reset it?
 
It may have carried over a restrictions code from iOS11. If he ever had a restrictions code setup on a previous install then try that.

Never had one. My husband is a basic user so never sets up anything like that. Any suggestions on how to fix it or get rid of it?
 
Off the top of my head, I have no idea. The code it asks for - is it 4 digits or 6 digits?

The 4 digit code is a restriction code. The 6 digit code is your PIN.

I know these questions might sound pointless, but may point us in the right direction to working it out.

It is a 4 digit code.

This came about after he upgraded to the iOS 12.

I had the same issue - I think it is blank at first and you can enter any 4 digit code as both the "Old Code" and then again as the new code.

At least this worked for me and the code is now accepted - now there is another code to remember and archive.
